Job summary

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and therapy needs.
• Develops and implements the therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observed and reports patient progress and changes to supervising clinician.
• Educates patient and family about plan of care and progress.
• Maintains documentation including progress notes and evaluations.
