Job summary

Apploi is seeking a licensed Physical Therapist to provide in-house skilled nursing facility services in Wilson, North Carolina. You will evaluate residents, design and implement treatment plans, and ensure CMS-compliant documentation within NetHealth EMR.

This PRN position requires a BS in Physical Therapy, an active license in the state, and commitment to ongoing professional development. You will collaborate with an interdisciplinary team to maintain high-quality resident care and drive

Qualifications

• BS in Physical Therapy required.
• Active Physical Therapy license in state of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents following standards of practice and company procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop and implement treatment plans with short- and long-term goals.
• Screen residents regularly to determine need for intervention/treatment.
• Communicate findings and plans to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
