Job summary

Trinity Rehab is seeking a dedicated Physical Therapist to join our team in Albemarle, NC on a PRN, part-time basis. You will evaluate patients, develop individualized treatment plans, and guide therapy to improve mobility and independence in a SNF setting.

Qualifications

• Licensed Physical Therapist in North Carolina.
• Experience in SNF or rehab setting preferred.
• Commitment to patient-centered care.

Responsibilities

• Evaluate and treat patients with individualized plans.
• Help patients improve mobility, strength and function.
• Monitor progress and adjust plans as needed.
• Collaborate with interdisciplinary team for quality care.
• Educate patients and caregivers on home programs.
• Maintain accurate documentation.
