Job summary

Evolve Therapy Services in Wilson, NC is seeking a dedicated Physical Therapist to join our in-house skilled nursing facility team. You will provide therapy services, develop treatment plans, and ensure documentation meets CMS standards while collaborating with the interdisciplinary team.

The ideal candidate has a BS in Physical Therapy, an active license, and a commitment to ongoing professional development.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents and interdisciplinary team.
• Complete CMS-compliant and medically necessary documentation in NetHealth EMR.
