Job summary

HealthMark Rehabilitation in The Woodlands, TX seeks PRN Physical Therapists to treat residents and short-term patients at Park Manor of the Woodlands facility in The Woodlands, Texas. Weekdays and weekends availability is desired.

Must hold Texas PT license; provide therapy per physician orders, document progress, and collaborate with an interdisciplinary team using state-of-the-art rehabilitation equipment.

Qualifications

• Must hold Texas PT license/certification and possess strong oral and written communication skills.
• Organizational and analytical skills required.

• Ability to utilize independent clinical judgment and problem-solving.

Responsibilities

• Provides therapy services per physician orders and professional standards.
• Develops an overall therapy plan of care with measurable goals.
• Performs skilled treatments according to the plan of care.
• Documents observations and patient response within required timeframes.
• Completes timely documentation per regulations and policies.
• Completes billing for provided services in accordance with regulations.
• Maintains rehab gym equipment and treatment area with infection control practices.
