Job summary

HealthMark Rehabilitation is seeking a Full-time Physical Therapist Assistant (PTA) to treat residents and short-term patients at Park Manor of Conroe in Conroe, Texas. This in-house therapy team serves HMG facilities.

The role requires Texas PTA license/certification, strong communication, and solid organizational skills. You will deliver skilled services per physician orders, document progress, and help maintain a safe rehab gym environment at the Conroe location.

Qualifications

• Texas license/certification as PTA required.
• Strong oral and written communication skills.

• Experience in geriatric rehabilitation preferred.

Responsibilities

• Provides therapy services per physician orders and accepted professional practices.
• Develops a plan of care with treatment duration, frequency, and measurable goals to support discharge.
• Performs skilled treatments according to the established plan of care.
• Documents observations and patient progress within required timeframes.
• Completes all required documentation in accordance with state and federal regulations.
• Maintains rehab gym equipment and treatment area in a safe manner with infection prevention practices.
