Job summary

HealthMark Rehabilitation is seeking a part-time CCC-SLP to treat residents at Park Manor of The Woodlands in The Woodlands, Texas. The role requires Texas licensure, strong communication, and organizational skills for therapy in a geriatric setting.

Responsibilities include delivering therapy per orders, documenting progress, and ensuring timely billing and safety in the rehab area. Join a inhouse therapy team serving SNF patients and residents.

Qualifications

• Must hold Texas license/ CCC-SLP certification.
• Strong oral and written communication skills; organizational and analytical skills required.
• Ability to function effectively in multi-faceted systems and to interact with all levels of staff and customers.
• Demonstrates time management and organization.
• Demonstrated ability to utilize independent and effective clinical judgment, problem-solving and decision making skills.
• Meet physical requirements.
• Experience in a geriatric rehabilitation setting preferred.
• Successful completion of hiring process / background check.

Responsibilities

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Registered therapists develop an overall therapy plan of care with treatment, goals, duration and frequency, to support discharge planning.
• Therapists of all disciplines perform skilled treatments according to the plan of care.
• Documents observations, patient progress and changes within specified timeframes.
• Completes all documentation timely and per state/federal regulations and policies.
• Completes all billing on the date of the provided skilled services, in compliance with regulations.
• Assist with maintaining rehab gym equipment and treatment area in a safe manner with infection control.
