Qualifications

• CA license is required (or eligible)
• NPTE in Physical Therapy required
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification or willingness to obtain
• Experience in skilled nursing or post-acute care preferred

Responsibilities

• Perform comprehensive evaluations and reassessments per physician orders
• Develop and modify individualized plans of care with measurable goals
• Deliver skilled interventions to restore function and reduce impairment
• Supervise PTAs and rehab staff as allowed by law
• Document timely, accurate, and compliant progress notes and discharge planning
• Participate in care conferences, MDS, and quality initiatives
