Job summary

Pleasant Hill Post Acute in Pleasant Hill, CA, is seeking a compassionate Physical Therapist (PT) to join our in-house rehabilitation team on a PRN basis. Starting pay is $65+ per hour with flexible scheduling and opportunities to work with an interdisciplinary care team to achieve excellent resident outcomes.

Qualifications

• Must hold a California Physical Therapist license.
• Strong clinical and communication skills.
• Ability to work effectively within a multidisciplinary team.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy to improve mobility, strength and independence.
• Supervise PTAs and collaborate with the interdisciplinary care team.
• Participate in care planning and discharge preparation.
• Educate residents, families and caregivers.
• Maintain accurate and timely clinical documentation.
