Job summary

Pleasant Hill Post Acute, part of PACS, is looking for a compassionate Physical Therapist (PT) to join our in-house rehabilitation team in Pleasant Hill, CA. This PRN role offers flexible scheduling and competitive pay to support resident mobility and independence.

Responsibilities include evaluating residents, developing treatment plans, supervising PTAs, collaborating with the care team, and contributing to discharge planning. California PT license required.

• Strong clinical and communication skills.
