San Francisco Post Acute is seeking a PRN Physical Therapist to join our geriatric and post-acute rehabilitation team in San Francisco, CA. This per diem role offers a flexible schedule and competitive compensation, making it ideal for therapists seeking supplemental income or work-life balance.

Responsibilities include evaluating patients, developing treatment plans, documenting progress, collaborating with a multidisciplinary team, and educating patients and families on home exercise programs.

Qualifications

• Active, unrestricted Physical Therapist license in California.
• Current BLS/CPR certification.
• Experience in SNF, acute, or post-acute rehab preferred.

Responsabilités

• Evaluate patients and establish individualized treatment plans and goals.
• Administer evidence-based physical therapy interventions to improve mobility and function.
• Document progress and discharge plans per regulatory standards.
• Collaborate with physicians, OT, SLP, and nursing staff.
• Educate patients and families on home exercise programs and safety.
