The position requires licensure in the state and graduating from an accredited PT program, with coverage for documentation, communication with physicians, and ongoing plan-of-care updates.

Qualifications

• Graduate of an accredited physical therapy curriculum.
• Licensed in the state practicing by the Board of Physical Therapy and provide a Criminal Background Check.

Responsibilities

• Evaluate and design a plan of care for each patient.
• Perform timely patient evaluations and document progress.
• Document all services & communicate with physicians and POA as needed.
• Supervise PT assistants and rehabilitation techs as appropriate.
