Job summary

The PT will provide skilled therapy using evidence-based techniques, reassess progress periodically, and communicate findings to the medical team while ensuring compliance with insurance and documentation

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• Ability to pass criminal background check.
• Knowledge of applicable physical therapy practices, safety precautions, and insurance requirements.

Responsibilities

• Evaluate and treat patients referred by a physician to reduce pain, restore function, and improve functional independence.
• Develop individualized plans of care with measurable short and long-term goals.
• Provide skilled physical therapy using appropriate therapeutic techniques, modalities, and exercises.
• Re-evaluate patients regularly and modify treatment plans as needed.
• Monitor patient progress and communicate status to physicians.
• Complete accurate and timely documentation for clinical and billing purposes.
