Advocate Health in Charlotte, NC seeks PRN Physical Therapists to provide coverage across outpatient clinics in Greater Charlotte. Responsibilities include evaluating patients, creating care plans, and delivering education. Flexible schedules with advance notice.

Requires a Bachelor's Degree in Physical Therapy, valid North Carolina PT license, and CPR/BLS. Candidates should be able to work 50% weekdays, one Saturday monthly, and participate in other schedule needs across multiple sites.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• North Carolina PT license required.
• CPR Certification and BLS required per policy guidelines.

Responsibilities

• Evaluates patients and establishes care plans including modalities, procedures, exercises, and education.
• Gathers data from medical records, physician reports, patients and families to guide treatment.
• Documents evaluations, progress notes, daily treatment notes, and discharge planning.
• Re-evaluates patients as status changes and updates goals accordingly.
• Communicates evaluation results and progress to physicians, referrals and other staff; coordinates schedules and discharge planning.
• Provides patient and family education on deficits, progress toward goals, and home programs; assigns/ supervises PT assistants and staff.
