Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Active state licensure to practice Physical Therapy.
• CPR and BLS certifications required.

Responsibilities

• Evaluates patients and establishes appropriate patient care plan including modalities, procedures, exercise programs, and patient education.
• Gathers data from medical records, physician reports, patient and family; identifies need for assistive/orthotic devices.
• Documents evaluations, tests, progress notes, discharge notes, and related charges.
• Re-evaluates patients as needed and adjusts goals/treatment.
• Communicates evaluation and progress to physician, referral sources, and other staff; coordinates schedules and discharge planning.
