Job summary

Vertis Therapy is seeking a compassionate PRN Physical Therapist (PT) to join our rehabilitation team at Bloom at Eagle Creek, an assisted living community in Indianapolis, IN. The PT will develop individualized treatment plans and provide skilled services, supervising PTAs as needed.

Responsibilities include resident assessment, documentation, and collaboration with the interdisciplinary team to support residents’ independence and mobility. PRN hours are approximately 2 days per week.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid Physical Therapist state license or license eligibility.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a team environment.

Responsibilities

• Resident assessment and treatment: Evaluate residents' functional abilities and plan care to improve safety and independence.
• Documentation: Complete timely and accurate documentation in the EMR.
• Supervision: Provide guidance to Physical Therapy Assistants as appropriate.
• Professional development: Participate in team development meetings.
• Wellness programming: Lead community-wide wellness programs and events.
