Job summary

Vertis Therapy is looking for a passionate Physical Therapist for consistent PRN hours at our Assisted Living, Commons at Jackson Creek, in Bloomington, IN. The PT will develop individualized treatment plans, provide skilled physical therapy services, monitor resident progress, and collaborate with the interdisciplinary team.

The PT will also provide guidance and supervision to Physical Therapy Assistants (PTAs) as appropriate, documenting progress in the EMR and participating in wellness

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Active or eligible Physical Therapist license in Indiana.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Resident Assessment and Treatment: Evaluate residents' functional abilities and plan care to improve safety and mobility.
• Documentation: Complete timely and accurate documentation in the EMR.
• Supervision: Provide guidance to Physical Therapy Assistants as appropriate.
• Professional Development: Participate in or lead team development meetings.
• Wellness Programming: Lead community wellness programs and events.

The Physical Therapist will develop individualized treatment plans, provide skilled physical therapy services, monitor resident progress, and collaborate with the interdisciplinary team. The PT will also provide guidance and supervision to Physical Therapy Assistants (PTAs) as appropriate.

Responsibilities:

Resident Assessment and Treatment: Evaluate residents' functional abilities. Establish and execute a plan of care based on individual goals to improve resident's safety, functional mobility, and overall independence.

Documentation: Complete timely and accurate documentation.

Supervision: Provide appropriate guidance and supervision to Physical Therapy Assistants.

Professional Development: Participate or lead team development meetings.

Wellness Programming: Regularly lead community wide wellness programs and events.

Qualifications:

Education: Degree in Physical Therapy from an accredited institution.

Licensure: Valid Physical Therapist state license or license eligibility.

Skills: Strong clinical assessment skills. Proficient in developing and implementing effective treatment plans. Excellent communication and interpersonal skills. Ability to work independently and in a team environment.

Physical Requirements: Ability to lift and assist clients as needed. Must be able to stand for extended periods and perform physical tasks related to therapy.

Technology: Computer proficiency to document in electronic medical record platform.
