Job summary

Vertis Therapy is seeking a compassionate PRN Physical Therapist (PT) to join our rehabilitation team at Bloom at Eagle Creek, an assisted living community in Indianapolis, IN. The PT will develop individualized treatment plans and provide skilled services, supervising PTAs as needed.

Responsibilities include resident assessment, documentation, and collaboration with the interdisciplinary team to support residents’ independence and mobility. PRN hours are approximately 2 days per week.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Valid Physical Therapist state license or license eligibility.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a team environment.

Vertis Therapy is seeking a compassionate and dedicated PRN Physical Therapist (PT) to join our rehabilitation team at Bloom at Eagle Creek, an assisted living community in Indianapolis, IN. This position offers consistent PRN hours approximately 2 days per week, making it a great opportunity for a PT looking for a reliable schedule with the flexibility of PRN work. The community serves both assisted living and memory care residents, providing an opportunity to work with a diverse resident population and support individuals in maintaining their independence, safety, and functional mobility.

The Physical Therapist will develop individualized treatment plans, provide skilled physical therapy services, monitor resident progress, and collaborate with the interdisciplinary team. The PT will also provide guidance and supervision to Physical Therapy Assistants (PTAs) as appropriate.

Responsibilities:

Resident Assessment and Treatment: Evaluate residents' functional abilities. Establish and execute a plan of care based on individual goals to improve resident's safety, functional mobility, and overall independence.

Documentation: Complete timely and accurate documentation.

Supervision: Provide appropriate guidance and supervision to Physical Therapy Assistants.

Professional Development: Participate or lead team development meetings.

Wellness Programming: Regularly lead community wide wellness programs and events.

Qualifications:
