Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Valid state Physical Therapist license.
• Proficiency with Microsoft Office applications and basic internet navigation.

Responsibilities

• Provide physical therapy services including screening, evaluation, and treatment as referred by an approved referral source.
• Direct and supervise PTAs, Rehab Techs, and/or students in accordance with state practice acts and policies.
• Maintain professional conduct and adhere to regulatory guidelines governing physical therapy practice.
• Coordinate discharge planning and case management responsibilities with the rehab team.
• Document care plans, progress notes, and billing information accurately in the medical record.
• Maintain patient rights and confidentiality; act ethically and professionally at all times.
