Job summary

Soddy-Daisy Healthcare Center is seeking a licensed Physical Therapist Assistant to join our in-house therapy team. You will carry out individualized treatment plans, monitor progress, and collaborate with an interdisciplinary team to improve residents’ strength, mobility, and confidence.

In this PRN role, you’ll support the Director of Rehabilitation, supervise rehab technicians, document care, and help integrate PT services into facility plans while adhering to state guidelines and our quality

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Valid state Physical Therapy Assistant license.
• Proficiency with Microsoft Office and basic internet navigation.

Responsibilities

• Reports to the Director of Rehabilitation and Physical Therapist.
• Assist in screening and providing input to initial evaluations as appropriate.
• Implement treatment for all assigned patients in accordance with the established plan of care.
• Comply with state and federal guidelines regarding documentation and billing.
• Supervise rehabilitation technicians during group and individual treatment.
• Integrate physical therapy services into facility and interdisciplinary plans.
• Contribute to utilization review, care conferences, and team meetings.
• Maintain professional relationships with healthcare professionals and colleagues.
