Job summary

Edenbrook of Fond du Lac, part of Eden Senior Care, LLC, is seeking a PRN Physical Therapist Assistant to deliver rehabilitation services across service locations. The role requires Wisconsin PTA licensure, strong communication, and patient-care skills to support residents’ functional goals.

You will work under a physical therapist, contribute to treatment plans, and assist with discharge planning while maintaining accurate documentation and collaborating with the care team.

Qualifications

• Current Wisconsin PTA license in good standing.
• Degree from an accredited PTA program.
• Strong communication, documentation, and patient-care skills.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• Maintain rehabilitation standards in line with the facility's mission and goals.
• Provide quality therapy services under supervision of a PT.
• Support discharge planning and documentation.
• Collaborate with team to ensure consistent patient care and timely progress notes.
