Job summary

Eden Senior Care’s Edenbrook of Fond du Lac is seeking a PRN Physical Therapist to provide rehabilitation services at one of our skilled nursing facilities in Fond du Lac, WI. The PT will assess needs, develop and deliver therapy programs, and collaborate with the care team to promote patient independence.

Responsibilities include implementing individualized plans, coordinating referrals to other disciplines, training patients and caregivers, and supervising PT staff as needed.

Qualifications

• Proven work experience as a physical therapist.
• Current knowledge of treatment practices.
• Graduate degree or greater in physical therapy.
• Valid Wisconsin state license to practice physical therapy.

Edenbrook of Fond du Lac in Fond du Lac, WI a skilled nursing facility in the Eden Senior Care Network, is now hiring a PRN Physical Therapist to provide rehabilitation services in one or more of our service locations!

The Physical Therapist is responsible for assessing the need for, developing and delivering physical therapy programs to facilitate rehabilitation.

Primary Job Responsibilities:

• Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
• Identify need for and make referral to other disciplines to address the comprehensive needs of the patient
• Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathology
• Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards
