Job summary

Edenbrook Pine Haven, part of Eden Senior Care, is seeking a PRN Physical Therapy Assistant to provide high-quality rehab services in a skilled nursing facility. The PTA will operate under the supervision of a physical therapist, assist with evaluations, discharge planning, and ensure accurate documentation and billing.

Qualifications

• Active Minnesota PTA license in good standing.
• Degree from an accredited PTA program.
• Strong communication, documentation, and patient-care skills.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• Maintain rehabilitation standards per mission and goals.
• Provide quality therapy under supervision of a PT.
• Assist with discharge planning and related documentation.
• Screen patients for evaluation under PT supervision.
• Treat patients to relieve pain and restore function under PT supervision.
• Complete and timely documentation and billing summaries.
• Collaborate with the care team and participate in care plan meetings.
