Job summary

Eden Senior Care’s Edenbrook of Fond du Lac is seeking a PRN Physical Therapist to provide rehabilitation services at one of our skilled nursing facilities in Fond du Lac, WI. The PT will assess needs, develop and deliver therapy programs, and collaborate with the care team to promote patient independence.

Responsibilities include implementing individualized plans, coordinating referrals to other disciplines, training patients and caregivers, and supervising PT staff as needed.

Qualifications

• Proven work experience as a physical therapist.
• Current knowledge of treatment practices.
• Graduate degree or greater in physical therapy.
• Valid Wisconsin state license to practice physical therapy.

Responsibilities

• Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance.
• Identify need for and refer to other disciplines to address the patient’s comprehensive needs.
• Instruct, educate and train patients and caregivers to promote independence and productivity.
• Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements.
