American Senior Communities in Seymour, IN is seeking a PRN Physical Therapist Assistant (PTA) to provide skilled care under a licensed physical therapist. The role supports patients across ASC Therapies & Wellness settings including home health, SNF, and outpatient services.

The PTA will develop and continue treatment plans, assist with discharge planning, contribute to case management, and uphold state regulations while delivering compassionate care to senior residents across ASC facilities.

The Physical Therapy Assistant assists in providing skilled care under the supervision of the registered physical therapist. Able to provide services for patients in all ASC Therapies & Wellness Settings. Able to provide services for patients in all ASC Therapies & Wellness Settings including home health, skilled nursing, outpatient therapies.
