Job summary

Eden Senior Care, LLC is seeking a Physical Therapist (PT) for Edenbrook St. Cloud, a skilled nursing facility, to provide rehabilitation services. The role is available as full-time and/or PRN with a sign-on bonus.

The PT will assess, plan, and implement PT programs to support patient rehabilitation across service locations in Minnesota. The successful candidate will develop personalized therapy plans, coordinate referrals, educate patients and families, and supervise PT staff where applicable,

Qualifications

• Doctor of Physical Therapy or Master's in Physical Therapy from an accredited program.
• Active Minnesota Physical Therapy license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
• Identify need for and make referral to other disciplines to address the comprehensive needs of the patient
• Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity
• Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards

Position: Physical Therapist (PT) - Full-time and/or PRN

Edenbrook St. Cloud, a skilled nursing facility in the Eden Senior Care Network, is now hiring a Full-Time Physical Therapist to provide rehabilitation services in one or more of our service locations!

The Physical Therapist is responsible for assessing the need for, developing and delivering physical therapy programs to facilitate rehabilitation.

Primary Job Responsibilities

• Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
• Identify need for and make referral to other disciplines to address the comprehensive needs of the patient
• Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathology
• Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards

Qualifications

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program.
• Active Minnesota Physical Therapy (PT) license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.
