Job summary

Eden Senior Care is hiring a PRN Physical Therapist to provide rehabilitation services at Friendly Village Nursing & Rehab, a member of our network in Wisconsin. The clinician will assess needs, develop and deliver physical therapy programs, and help patients regain function in skilled nursing settings.

Qualified candidates hold a DPT or MPT and an active Wisconsin PT license, with experience in SNF, home health, or rehab settings preferred.

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (MPT) from an accredited program.
• Active Wisconsin Physical Therapy license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• Assess patient needs and design rehabilitation plans.
• Deliver PT services to restore function and mobility.
• Develop care plans using evidence-based techniques.
• Create individualized therapy programs for mobility and strength.
• Monitor progress and adjust treatments as needed.
• Document patient care activities and outcomes.
• Collaborate with other health professionals to optimize care.
• Promote functional independence and safety for patients.

Position: PRN Physical Therapist (PT)

Friendly Village Nursing and Rehab a skilled nursing facility in the Eden Senior Care Network, is now hiring a PRN Physical Therapist to provide rehabilitation services in one or more of our service locations!

The Physical Therapist is responsible for assessing the need for, developing and delivering physical therapy programs to facilitate rehabilitation.

Responsibilities

• Identify and meet patients’ goals and needs
• Offer cost-effective treatments that help improve clients’ motion and mobility
• Reduce the need for medications and provide alternatives to surgery
• Develop care plans using a variety of treatment techniques
• Create fitness- and wellness-oriented programs tailored to patients’ specific needs
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
• Consult and practice with other health professionals
• Evaluate effects and monitor and communicate progress
• Document patient care services
