Job summary

Eden Senior Care, LLC’s Edenbrook Oshkosh is hiring a PRN Physical Therapist to provide rehabilitation services in our skilled nursing facility.

The Physical Therapist will assess needs, develop and deliver therapy programs, supervise staff, and collaborate with caregivers to promote patient independence and recovery at this Wisconsin site.

Qualifications

• DPT or MPT from an accredited program.
• Active Wisconsin PT license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• Develop and implement individualized therapy programs.
• Identify referrals to other disciplines to address patient needs.
• Instruct, educate and train patients and caregivers to promote independence.
• Supervise PTAs, provisional licensees, students, and support personnel per licensure rules.

Position: Physical Therapist (PT)

Edenbrook Oshkosh a skilled nursing facility in the Eden Senior Care Network, is now hiring a PRN Physical Therapist that can provide rehabilitation services at our facility.

The Physical Therapist is responsible for assessing the need for, developing and delivering physical therapy programs to facilitate rehabilitation.

Primary Job Responsibilities

• Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
• Identify need for and make referral to other disciplines to address the comprehensive needs of the patient
• Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathology
• Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards
• Develop care plans using a variety of treatment techniques
• Create fitness- and wellness-oriented programs tailored to patients' specific needs
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
• Evaluate effects and monitor and communicate progress

Qualifications

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program.
• Active Wisconsin Physical Therapist (PT) license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.
