Job summary

PruittHealth is seeking an Occupational Therapist (OT) for Home Health Services in North Carolina, Raleigh area. The role involves evaluating and treating homebound patients referred by a physician, and coordinating care with the clinical team.

Applicants should hold an ACOTE-accredited OT degree, proper state licensure, and be able to complete required documentation and assessments such as OASIS within standard timeframes.

Qualifications

• Must have a valid and unrestricted professional OT license in state of practice.
• Graduate of ACOTE accredited school.
• If foreign-trained, credentials must be substantially equivalent to US OT entry level education.

Responsibilities

• Assesses, monitors, documents and reports patient progress using clinician tools and required timelines.
• Collaborates with interdisciplinary team to develop and manage the plan of care.
• Demonstrates competency in home health care and patient care skills.
• Completes timely, accurate documentation (e.g., OASIS, SOC/Admissions, Recertifications).
• Maintains safety regulations and adheres to agency guidelines.
