Responsabilités

• Assesses, monitors, documents and reports patient progress using required documents and timelines.
• Collaborates with the interdisciplinary team to develop and manage the patient care plan.
• Completes documentation timely and accurately (e.g., OASIS, SOC/Admissions, recertifications).
