Job summary

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of ACOTE accredited school.
• If foreign-trained, the program must be deemed substantially equivalent by a credentials evaluation organization approved by AOTA.

Responsibilities

• Assess, monitor, document, and report patient health and condition within established timelines.
• Collaborate with the interdisciplinary team to develop and manage the care plan.
• Demonstrate home health competency and appropriate patient care skills.
• Prioritize patient care needs with initiative and sound judgment.
• Assist with training and orienting others.
• Utilize teaching materials for patient/family education.
• Complete documentation timely and accurately at the point of care (OASIS, SOC/Admissions, etc.).
• Report any violations to supervisor and ensure resolution.
• Maintain compliance with corporate program and productivity standards.
• Participate in surveys, in-services, and care conferences; support safety and quality initiatives.
