Mallard Creek in Fort Worth, TX seeks a PRN Occupational Therapist to join our in-house rehab program serving geriatric clients in post-acute and long-term care. You will screen and treat patients with cognitive and ADL disorders and develop individualized plans of care.

We value licensed OT with flexibility; duties include using standardized assessments, delivering rehabilitative interventions, documenting progress, and coordinating discharge summaries.

Qualifications

• Licensed Occupational Therapist
• Open to applicants of all experience levels

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders; develop plan of care per standards.
• Utilize standardized assessments and evidence-based practice.
• Provide rehabilitative, skilled, and medically necessary treatment per plan.
• Administer procedures as part of rehabilitation plan (ADL retraining; therapeutic activities; exercises; modalities; devices).
• Assess progress and adjust treatments in clinical records per requirements.
• Evaluate treatment outcomes and modify services; refer as needed.
• Provide counseling to patients and families and other providers.
• Communicate progress, barriers, and plans with supervisor and health team.
• Provide comprehensive discharge summaries per regulatory requirements.
