Flagship Therapy in Willow Park, TX is seeking a PRN Occupational Therapist for our in-house rehab program. You will screen and treat patients with cognitive and ADL impairments, develop plans of care, and collaborate with the rehab team.

Open to applicants of all experience levels; licensed OT required. You will join a values-driven team focused on high-quality geriatric care and ongoing education.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL functioning disorders and develop plan of care following standards.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per requirements.
• Administer procedures as part of rehabilitation plan (ADL retraining, therapeutic activities, exercises, modalities, assistive devices).
• Assess progress and adjust treatments in clinical records.
• Evaluate treatment outcomes and modify services; refer as needed.
• Provide consultation to patients, families, caregivers and other providers.
• Communicate progress and plans with supervisor and health team.
• Provide comprehensive discharge summaries per regulations and requirements.
