Job summary

Evolve Therapy Services, LLC in Humboldt, TN is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility on a PRN basis. You will implement the established plan of care under supervision, help determine patient needs, and document progress according to CMS standards.

The ideal candidate holds an active OTA license in the state(s) of practice and an associate degree in Occupational Therapy, with commitment to ongoing professional development and quality patient

Qualifications

• Must have OTA licensure active in state(s) of practice.
• Associate's degree in Occupational Therapy required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment plan.
• Execute treatment plans and monitor progress toward short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish individual resident therapy schedules with team members.
• Document therapy and ensure billing aligns with plan of care.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.
