Job summary

Evolve Therapy Services, LLC in Newport, TN, seeks a Certified Occupational Therapy Assistant to deliver in-house therapy at our partner SNF.

You will plan and implement treatment per plan of care, schedule therapies with the team, document outcomes, and ensure CMS compliance.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention and treatment.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings and recommendations to residents and team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy and ensure proper documentation and billing of occupational therapy services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.
