Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Church Hill, TN. This PRN position requires an active OTA license and an Associate's degree in Occupational Therapy to plan, organize, develop, and direct therapy services in a facility setting.

Responsibilities include screening residents, implementing treatment plans, coordinating therapy schedules, documenting care, and ensuring compliance with CMS standards.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment per scope of practice.
• Perform treatment plans and follow short- and long-term goals from the plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families.
• Establish individual resident therapy schedules with team members.
• Care for residents according to the treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice
