Job summary

Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Newport, TN. This PRN position focuses on delivering occupational therapy services per plan of care and collaborating with a multidisciplinary team to ensure quality resident care.

Responsibilities include screening residents, implementing treatment plans, documenting progress, and coordinating schedules while adhering to CMS standards and approved protocols.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice.
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
