Job summary

Creative Solutions In HealthCare seeks a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will assess residents and develop individualized treatment plans to improve independence, safety, and quality of life in daily activities and IADLs.

The role requires Texas licensure and preferred SNF/long-term care experience; Net Health experience is a plus. Flexible scheduling is available to accommodate residents' needs.

Qualifications

• Active Texas Occupational Therapy license is required.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize performance in ADLs and IADLs.
• Assess patients' home and living environments and recommend modifications.
• Provide training on adaptive equipment to support independence and safety.
• Ensure treatments are delivered according to the plan of care.
• Monitor patient responses and adjust interventions with the team.
• Provide clinical education and support to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely per policies and regulations.
• Attend required meetings as designated by the Director of Rehab
