HealthMark Rehabilitation is seeking a PRN Occupational Therapist/OTR to treat residents and short‑term patients at Willowbrook Nursing and Rehabilitation in Nacogdoches, TX. You will provide skilled therapy aligned with physician orders and discharge plans, working within a large interdisciplinary team.

The role requires Texas licensure (OTR) and strong communication, organizational and analytical skills. Prior geriatric rehab experience is preferred; you will complete the hiring process and

Formación

• Must hold Texas license/certification for the Occupational Therapist Registered (OTR).
• Experience in geriatric rehabilitation preferred.
• Demonstrates ability to function in multi-faceted systems and interact with all levels of staff and customers.
• Successful completion of hiring process / background check.

Responsabilidades

• Provides therapy services in accordance with physician’s orders and accepted professional practices.
• Develops an overall plan of care with treatment, goals, duration and frequency to support discharge.
• Performs skilled treatments according to the plan of care.
• Documents observations and patient responses within designated timeframes.
• Completes all documentation timely and in accordance with state and federal regulations.
• Assists with maintaining rehab gym equipment and treatment area safely, following infection prevention methods.
