Job summary

Putnam Nursing & Rehabilitation Center is seeking a PRN/Per Diem Occupational Therapist to join our skilled nursing facility rehabilitation team. You will evaluate and treat geriatric patients to improve ADLs, strength, and independence, collaborating with nurses, physicians, and therapists.

Qualifications

• Must possess a current license and registration to practice as an Occupational Therapist (OT) in NY.
• Must possess current malpractice insurance.
• Dedicated to providing extraordinary compassionate care as an Occupational Therapist (OT).
• Energetic, enthusiastic, committed and passionate about quality care.
• Genuinely interested in helping ill, injured, elderly residents.
• Patience, cheerful disposition and willingness to assist residents and co-workers.
• Excellent communication, customer service, and organizational skills.
• Trusted to work independently and in a team environment.
• Able to work well under pressure, flexible and harmonious with staff.

Responsibilities

• Licensed OT providing PRN rehabilitation services.
• Evaluate and treat geriatric patients with complex medical, cognitive and functional needs.
• Develop individualized treatment plans to improve mobility and daily living skills.
• Collaborate with nurses, physicians, SLPs and PTs for coordinated care.
• Skilled in adaptive equipment training, fall prevention and patient education.
• Provide high-quality care across variable schedules and patient acuity levels.
