Job summary

Patewood Post Acute in Greenville, SC is seeking a licensed Occupational Therapist (OTR) to provide patient-centered care on a PRN basis. You will evaluate residents, develop individualized plans, and deliver therapy services per physician orders at our 120-bed skilled nursing and rehabilitation facility.

You will document and communicate progress, collaborate with physicians and the care team, and educate residents and families about therapy programs while supporting care and discharge

Qualifications

• Must hold an active South Carolina Occupational Therapist license (OTR) in good standing.
• Experience in skilled nursing or rehab settings preferred.
• Strong documentation and communication skills.
• Ability to collaborate with physicians, nursing, and interdisciplinary teams.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled OT services per physician orders.
• Document evaluations, treatments, and progress notes in compliance with regulations.
• Collaborate with physicians, nursing, and interdisciplinary team members.
• Educate residents, families, and caregivers on therapy programs.
• Participate in care planning, discharge planning, and team meetings.
