Job summary

Powdersville Post Acute in Easley, SC is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team on a PRN basis. You will evaluate residents, develop individualized treatment plans, and help residents achieve their highest level of independence and quality of life.

Collaborating with an interdisciplinary team, you will provide therapy services per physician orders, document evaluations and progress notes, and participate in discharge planning and care conferences.

Qualifications

• Bachelor's degree in Occupational Therapy required.
• Master's or Doctorate degree in Occupational Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with Casamba and PointClickCare (PCC) preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services per physician orders and established goals.
• Document evaluations, treatment sessions, progress notes, and discharge summaries accurately.
• Monitor resident progress and modify treatment plans as appropriate.
• Communicate with physicians, nursing staff, residents, families, and other healthcare professionals regarding treatment plans and outcomes.
• Participate in care conferences, interdisciplinary meetings, and discharge planning.
• Educate residents, families, and caregivers on therapy goals and home programs.
• Conduct in-service training and education for facility staff as needed.
• Ensure compliance with state practice acts, facility policies, and regulatory requirements.
• Support residents in achieving their highest level of function, independence, and quality of life.
