Job summary

Enhance Rehabilitation is currently seeking a qualified Occupational Therapist to join our growing team in Fond du Lac, Wisconsin. In this SNF setting, you will evaluate and treat patients, develop plans of care, and set measurable functional goals.

You will document evaluations and progress notes, collaborate with facility staff, educate patients and families, and supervise Occupational Therapy Assistants to ensure excellent patient care.

Qualifications

• OT clinical assessment and treatment experience.
• Experience documenting evaluations and progress notes.
• Ability to supervise Occupational Therapy Assistants and collaborate with facility staff.

Responsibilities

• Evaluate and treat patients in a SNF setting following a plan of care.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
• Collaborate with facility staff to ensure patient care quality.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
