Job summary

Enhance Rehabilitation is seeking a licensed Occupational Therapist to join our SNF team. You will evaluate and treat patients, develop plans of care, set functional goals, and educate patients and families about injuries and outcomes.

You will collaborate with facility staff, re-evaluate progress, adjust plans of care, complete timely documentation, and supervise Occupational Therapy Assistants.

• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
