Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a skilled nursing facility setting. You will evaluate and treat patients, develop plans of care, and set measurable functional goals to improve independence and safety.

You will document evaluations, progress notes, discharges, and collaborate with facility staff. The role includes supervising Occupational Therapy Assistants and contributing to a positive, patient-centered rehabilitation program.

Responsibilities

• Providing excellent care for our patients
• Collaborate with facility staff to ensure excellent care for our patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
