Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and set measurable functional goals to guide progress.

Collaborate with facility staff, educate patients and families about injury and prognosis, re-evaluate progress, and maintain timely documentation. You will also supervise Occupational Therapy Assistants to ensure high‑quality care.

Responsibilities

• Evaluate new patients and develop plans of care with measurable functional goals
• Treat patients and educate them and their families on progress and outcomes
• Re-evaluate patient progress and revise plans of care as needed
• Complete timely documentation including evaluations, re-evaluations and daily progress notes
• Supervise Occupational Therapy Assistants and collaborate with facility staff to ensure quality care

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re‑evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
