Job summary

Hill Country Nursing and Rehabilitation is seeking a compassionate Occupational Therapist (OT) to join our in-house team on a PRN basis. You will evaluate residents, develop individualized ADL/IADL plans, and collaborate with an interdisciplinary team to improve independence and safety.

Must hold an active Texas OT license and may require SNF or long-term care experience. Familiarity with Net Health EDS is preferred, with strong communication and clinical decision-making skills essential for

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Proficiency with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home and living environments and recommend modifications.
• Recommend adaptive equipment and provide training to support independence and safety.
• Complete documentation thoroughly and timely in compliance with policies and regulations.
• Collaborate with interdisciplinary team to ensure high-quality patient care.
