Job summary

Five Points Nursing and Rehabilitation of Pflugerville is seeking a compassionate Occupational Therapist (OT) to provide PRN clinical care focused on improving residents’ independence and safety in daily activities. You will assess, plan, and implement therapies to maximize ADLs and IADLs while collaborating with the rehab team.

The role requires current Texas OT licensure, experience in SNF or long-term care, and the ability to educate patients and families.

Qualifications

  • Active Texas Occupational Therapy license required.
  • SNF or long-term care experience preferred.
  • Experience with Net Health Electronic Documentation System preferred.
  • Strong communication and collaboration skills.
  • Ability to adapt to diverse patient needs and personalities.

Responsibilities

  • Evaluate patients and determine impact on ADLs/IADLs.
  • Develop individualized treatment plans to maximize independence.
  • Assess home environments and recommend safety modifications.
  • Recommend adaptive equipment and train patients for use.
  • Deliver treatments per care plan and document progress.
  • Collaborate with interdisciplinary team and attend meetings.
  • Provide patient, family, and caregiver education.
