Job summary

Enhance Rehabilitation seeks an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop plans of care, and set measurable functional goals.

You will collaborate with facility staff, educate patients about their injury and progress, and supervise Occupational Therapy Assistants while ensuring high-quality patient care and documentation.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing concerns and educating them on injuries, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Completing timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborating with facility staff to ensure excellent patient care
• Providing excellent care for our patients and outstanding customer service to patients and family members
