Enhance Rehabilitation is seeking an Occupational Therapist to join our SNF team. You will evaluate and treat patients, develop personalized care plans, and set measurable goals to improve function.

You will collaborate with facility staff, provide patient and family education, document progress, and supervise Occupational Therapy Assistants to ensure high-quality care.

Qualifications

• Licensed Occupational Therapist with eligibility to practice in the state.
• Evaluates and treats patients in SNF following an individualized plan of care.
• Coordinates care with other staff to meet patient goals.

Responsabilités

• Provide evaluation and treatment to SNF patients.
• Develop and implement plans of care with measurable goals.
• Collaborate with facility staff for quality patient outcomes.
• Document evaluations, progress notes, and discharges timely and accurately.
• Supervise Occupational Therapy Assistants.
• Educate patients and families on injury, progress and prognosis.
• Re-evaluate progress and revise plans as needed.

Enhance Rehabilitation is currently seeking qualified Occupational Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
